Job Description This position provides community-based care coordination and mental health skill-building services to homeless individuals with serious mental illness and substance use disorders. Candidates must have a Bachelor's degree in Social Work, Counseling or Psychology and a minimum of one year experience in the field. Candidates must meet Qualified Mental Health Professional criteria and demonstrate skills in assessment, counseling, crisis management, and clinical documentation.
